Assisted Living Services is a program designed to assist seniors to continue to live in their homes. Services are based on individual needs. This program offers a combination of personal support, homemaking services, security checks and care coordination.

The following are the assisted living services available to eligible seniors:

  1. Personal support services – including personal hygiene, dressing, assist with mobility, assistance with premeasured medications and reminders and other activities of daily living. These services are available 24 hours a day, 7 days a week, on a scheduled and unscheduled basis.
  2. Security checks or reassurance services – these services shall be provided to address the individual needs of clients based on their clinical condition or environment. You may want to purchase services from a business that gives you a personal alarm to wear, in case of an emergency and cannot reach the phone. These services are available 24 hours a day, 7 days a week, on a scheduled and unscheduled basis.
  3. Care coordination – makes sure all the people who help look after you are working together. Care coordination will link you with other community service’s (e.g. meals on wheels or adult day programs): to care provided by primary health care professionals (e.g. general practitioners, family health teams, geriatric and psychogeriatric services, mental health services): and/or connecting you to agencies providing the equipment you might need, such as a wheelchair, walker or oxygen.
  4. Homemaking services – including light housecleaning, shopping and meal preparation that are necessary to maintain people in their own homes which they are unable to perform for themselves. These services are available 7 days a week on a scheduled basis and unscheduled basis, (unscheduled requests will be dependent on availability of staff).

INDIVIDUALS APPROPRIATE FOR SERVICES MUST RESIDE WITHIN THE GEOGRAPHIC AREA IN WHICH THE SERVICES ARE OFFERED. PRESENTLY OPERATING IN THE NORTH EAST END OF BARRIE, ONTARIO.

A FORMAL ASSESSMENT IS REQUIRED THROUGH THE NSM CCAC TO ESTABLISH ELIGIBILITY.
